FORM currently operates lab partnerships in Indonesia and Australia. An NZ-local lab partnership is coming. Until it lands, NZ men have two paths: combine the draw with a trip to Bali or one of our AU partner labs (Sydney, Melbourne, Brisbane, Perth), or use the remote-consult path where we provide a pathology referral your NZ GP can use locally.

Panel: total and free testosterone, SHBG, sensitive estradiol, LH, FSH, prolactin, DHEA-S, AM cortisol, full lipid and metabolic markers, HbA1c, fasting insulin, and key inflammatory markers.

If TRT is indicated, you receive an expert-reviewed PDF report with reference ranges and marker context your Tauranga GP can use. Follow-up panels can be done at any Bay of Plenty pathology lab.
