Before you commit to any Auckland clinic or consult, get accurate baseline numbers. FORM is a pathology testing service — we run an accredited hormone and metabolic panel and explain what your results mean, so you walk into any conversation about testosterone with real data.

FORM currently operates accredited lab partnerships in Indonesia and Australia. An NZ-local lab partnership is coming. Until it lands, NZ men have two paths to get tested: combine the draw with a trip to Bali or one of our Australian partner labs, or use the remote path where we help you interpret a panel your NZ doctor orders locally.

Our panel covers total and free testosterone, SHBG, sensitive estradiol, LH, FSH, prolactin, DHEA-S, AM cortisol, full lipid and metabolic markers, HbA1c, fasting insulin, and key inflammatory markers — the same baseline a competent NZ endocrinologist would want before discussing anything further.

What you get back is a plain-English, expert-reviewed report explaining what each marker means and which results are worth taking to a doctor. It is not a protocol and not a treatment plan — testosterone is a prescription medicine and only a New Zealand-registered doctor can assess whether it's right for you.

Can I take FORM's results to my Auckland GP?
Yes. The report is designed to be doctor-readable, but it is a testing and education product, not a referral or prescription pathway. Your GP makes their own independent assessment.
Is testosterone legal to prescribe in New Zealand?
Yes — testosterone is a prescription medicine that a New Zealand-registered doctor can prescribe after their own clinical assessment. FORM does not prescribe, supply or arrange testosterone.
How much does testing cost?
Pricing is in NZD on request. Privately requested pathology in New Zealand is not publicly funded — see the funding notice below for how that differs from a doctor-requested test.
