Where can I get a blood test in Bali?
FORM runs certified nurse home draws across Canggu, Seminyak, Ubud, Uluwatu, Sanur and Denpasar. The nurse comes to your villa, hotel or office — there is no clinic visit.
Do I need an appointment?
Yes. Draws are scheduled in advance on WhatsApp after you order, and run during normal business hours, 08:00–18:00 Bali time (WITA).
How long does the appointment take?
About 10 minutes from the nurse arriving to leaving. The nurse brings all equipment and tubes; you need photo ID and a sleeve that rolls above the elbow.
Do I need to fast?
Fast 10 hours for lipids, fasting glucose and fasting insulin — water only. Testosterone, thyroid, HbA1c and full blood count do not require fasting, but testosterone should be drawn before 10:00.
Can I cancel or reschedule?

How much does a blood test cost in Bali with FORM?
Panels run from Rp 3,299,000 for the Essential panel (21 biomarkers) to Rp 31,350,000 for the Ultra panel (68 biomarkers). A 2% service fee is added at checkout and shown as a line item.
What is included in the price?
The nurse home draw, laboratory processing, the results dashboard, an expert-reviewed PDF report, and an itemised receipt with ICD-10 codes for insurance.
Can I choose individual biomarkers?
Yes. The custom builder prices each biomarker individually and the cart itemises every marker you add, so you can adjust the panel before checkout.
Which payment methods does FORM accept?
Card or bank transfer, charged in IDR. Pay in 3 instalments is available at checkout on eligible orders.
Results and reports
How long do results take?
7–14 days from the draw. Results appear in your FORM dashboard and as a downloadable PDF report.
Who reviews the results?
The interpretation on every report is reviewed by Dr. Nikola Topalovic, MD PhD, FORM's Medical Director.
Does FORM prescribe treatment?
No. FORM is a pathology testing service. The report explains what each marker measures and flags what is worth discussing; prescribing is done by your own doctor.
Can I share the report with my own doctor?
Yes. The PDF carries biomarker values, units and reference ranges, and is designed to be forwarded to a GP or specialist.

Can I claim the test on insurance?
Every panel comes with an itemised receipt and ICD-10 codes for an international insurance claim. Customers on expat plans report averaging around 95% reimbursement, though the outcome depends on the individual policy.
Which insurers have reimbursed FORM panels?
