The five steps, start to finish
1.Choose a panel or build your own
Pick one of the four FORM panels, or select individual biomarkers in the custom builder. You add it to the cart and pay by card or bank transfer; Pay in 3 instalments is available on eligible orders.
2.Book your appointment time
FORM contacts you on WhatsApp to schedule the draw in advance. Appointments run inside normal business hours, 08:00–18:00 Bali time (WITA). Hormone panels are booked in the early morning because testosterone peaks between 07:00 and 10:00.
