Estradiol (Sensitive) (E2)
Estradiol is the active oestrogen in men, made primarily by aromatising testosterone in fat tissue. Use the sensitive (LC-MS/MS) assay — the standard immunoassay is unreliable at male concentrations. Too high causes gyno, water retention, mood issues; too low crashes libido and bone density.

What does a high Estradiol (Sensitive) result mean?
Above 150 pmol/L: obesity, alcohol, TRT without monitoring, rare aromatase-secreting tumours.
What does a low Estradiol (Sensitive) result mean?
Below 60 pmol/L: aggressive aromatase inhibitor use, low body fat, low testosterone. Causes joint pain, low libido, bone loss long-term.
Why does Estradiol (Sensitive) matter?
The T:E2 ratio matters more than either number alone. Men need oestradiol — it's not the enemy. The goal is appropriate range, not zero.
