A prolactin blood test measures the pituitary hormone that drives milk production, reported in mIU/L — and because prolactin rises with stress, exercise, sleep, nipple stimulation and a long list of common medicines, a single mildly raised result should always be repeated on a rested morning sample before anyone acts on it.

What a prolactin blood test measures

Prolactin is a hormone produced by lactotroph cells in the anterior pituitary, best known for driving milk production, and unusual among pituitary hormones in being held down rather than switched on — dopamine from the hypothalamus continuously suppresses it.

That inhibitory control explains almost everything clinically interesting about prolactin. Anything that reduces dopamine's reach to the pituitary raises prolactin: dopamine-blocking medicines, a stalk lesion interrupting the connection, or a lactotroph tumour outgrowing the signal. This is the opposite arrangement to most pituitary hormones, and it is why so many everyday medicines raise prolactin as a side effect.

Beyond lactation, prolactin exerts negative feedback on gonadotrophin-releasing hormone. When prolactin is high, GnRH pulses slow, LH and FSH fall, and the downstream consequences follow: irregular or absent periods and anovulation in women, and low testosterone with reduced libido in men. This is why prolactin appears in fertility panels, in the investigation of low testosterone, and in the work-up of erectile dysfunction.

Australian laboratories report prolactin in milli-international units per litre. US sources use ng/mL, and the approximate conversion is 1 ng/mL ≈ 21 mIU/L — so a US value of 20 ng/mL is about 420 mIU/L. Mixing units is a common source of unnecessary alarm.

Why collection conditions matter so much

Prolactin is a stress hormone as well as a lactation hormone, and the 30 minutes before the blood draw can change the result enough to alter what happens next.

Prolactin rises with venepuncture stress, physical exertion, a large meal, sleep, sexual activity, breast or nipple stimulation, and even a difficult or repeated needle attempt. It follows a sleep-related rhythm, peaking during sleep and falling through the morning, so a sample taken immediately on waking reads higher than one taken three hours later.

The standard approach in Australian practice is a morning sample taken at least an hour after waking, with the person seated quietly for 20 to 30 minutes beforehand, and without preceding exercise or breast stimulation. Where a first result is only mildly raised, repeating under those conditions resolves a substantial proportion of cases without any further investigation.

  • Collect mid-morning, at least one hour after waking — not immediately on rising.
  • Sit quietly for 20 to 30 minutes before the draw.
  • Avoid vigorous exercise, sexual activity and nipple stimulation that morning.
  • Mention any recent breast examination — even that can raise the level.
  • Bring a complete medicine list, including over-the-counter anti-nausea preparations.

Which panel includes prolactin

Prolactin belongs to the pituitary group and is read alongside the gonadotrophins and the sex hormones they control.

What a high prolactin can indicate

Hyperprolactinaemia is most often caused by medicines, physiological factors or macroprolactin, and less often by a pituitary adenoma — but the degree of elevation guides which is likely.

Medicines lead the list by a wide margin. Antipsychotics — particularly risperidone, paliperidone, amisulpride and haloperidol — are the strongest offenders. Metoclopramide and prochlorperazine, both widely used for nausea, raise prolactin substantially. Some antidepressants, verapamil, oestrogens and opioids contribute more modestly. None of these should be stopped or changed to chase a laboratory value; that decision belongs with the prescriber.

Primary hypothyroidism raises prolactin because elevated TRH stimulates lactotrophs, which is why TSH is checked alongside. Chronic kidney disease reduces prolactin clearance. Chest wall injury, shingles affecting the chest and even tight clothing over the chest can raise it neurogenically. Pregnancy and breastfeeding raise it physiologically, and pregnancy is always excluded first in a woman of reproductive age.

Macroprolactin deserves specific mention. In some people, prolactin circulates bound to immunoglobulin as a large complex that assays detect but that is biologically inactive. It causes no symptoms and needs no treatment, yet produces persistently raised results. Australian laboratories screen for it — usually by polyethylene glycol precipitation — when prolactin is elevated without a clear explanation. Ask whether macroprolactin was assessed before accepting a raised result at face value.

Prolactinoma is the pathological cause that matters. It is the commonest functioning pituitary tumour, is usually small and benign, and the level tends to track with size — which is why markedly raised values prompt MRI. Very large tumours can occasionally produce falsely low readings through assay saturation (the 'hook effect'), so a large lesion with an unremarkable prolactin is re-tested on a diluted sample.

What a low prolactin means

Low prolactin is rarely clinically significant on its own and is usually only relevant when the whole pituitary is underactive.

There is no meaningful lower limit to chase. A low prolactin matters where hypopituitarism is suspected — after pituitary surgery, radiotherapy, head injury, or in Sheehan syndrome following major postpartum haemorrhage — and in that setting it is one component of a full anterior pituitary assessment rather than a finding in itself. Dopamine agonists lower prolactin by design.

Who should consider a prolactin test

Prolactin is tested where the reproductive axis is behaving abnormally, where there is milk discharge outside breastfeeding, or where pituitary symptoms are present.

  • Irregular, infrequent or absent periods, or unexplained anovulation.
  • Milky nipple discharge (galactorrhoea) outside pregnancy or breastfeeding, in either sex.
  • Subfertility in women or men, as part of a fertility panel.
  • Low testosterone in men, particularly with low or normal LH — a low-LH pattern makes prolactin essential.
  • Reduced libido or erectile dysfunction alongside other endocrine symptoms.
  • Headache with visual field disturbance, which raises the possibility of a pituitary lesion — this warrants prompt medical assessment, not just a blood test.
  • Monitoring where a doctor has identified hyperprolactinaemia and is following it.

What is a normal prolactin level in Australia?
Indicatively, about 60–400 mIU/L in adult men and 100–500 mIU/L in non-pregnant adult women. Intervals vary between assays, so read your result against the range printed on your own report.
What does a high prolactin mean?
Most often a medicine, a physiological rise from stress, exercise or sleep, or macroprolactin — an inactive bound form the assay detects. Less often hypothyroidism, chronic kidney disease or a pituitary adenoma. The higher the level, the more likely a pituitary cause, with values above about 2,500 mIU/L usually prompting imaging.
Do I need to fast for a prolactin test?
No, but preparation still matters. Collect mid-morning at least an hour after waking, sit quietly for 20 to 30 minutes first, and avoid exercise, sexual activity and nipple stimulation beforehand.
Why does my prolactin need repeating?
Because a large share of mild elevations are physiological or collection-related. Repeating under proper rested morning conditions, with macroprolactin screening, resolves many raised results without any further investigation.
What is macroprolactin?
Prolactin bound to immunoglobulin as a large, biologically inactive complex. Assays detect it, so it produces a persistently raised result in someone with no symptoms and no underlying problem. Australian laboratories screen for it when prolactin is unexpectedly high.
Which medicines raise prolactin?
Antipsychotics such as risperidone, paliperidone, amisulpride and haloperidol are the strongest; metoclopramide and prochlorperazine also raise it substantially.
