Zinc Blood Test in Australia

A zinc blood test measures the concentration of zinc in plasma or serum — the standard laboratory marker of zinc status — used to investigate suspected deficiency in people with poor intake, malabsorption or increased losses.

What a zinc blood test is

A zinc blood test measures the zinc concentration in plasma or serum, which is the practical laboratory marker of zinc status even though only a small fraction of total body zinc circulates.

Zinc is a trace element required by hundreds of enzymes and by the zinc-finger proteins that regulate gene transcription. It is essential for immune function, wound healing, taste perception, skin integrity, growth and reproduction. The body holds around two to three grams in total, most of it inside muscle and bone, with well under one per cent circulating in plasma.

That distribution is the central interpretive caveat. Plasma zinc is a small, tightly regulated pool, and it responds to things other than intake. About 80% of circulating zinc is bound to albumin, so a low albumin — from liver disease, nephrotic syndrome, pregnancy or simply lying down for a while before collection — drags plasma zinc down with it. Inflammation does the same thing more sharply: during an acute-phase response, zinc is redistributed into the liver and plasma levels can fall by a quarter or more within a day, independent of body stores.

Collection technique also matters more than for routine chemistry. Zinc is present in rubber stoppers, some tube coatings and in ambient dust, so a trace-element tube is required. Levels also follow a daily rhythm, falling after meals and through the afternoon, which is why Australian laboratories specify a fasting morning sample.

Read with those caveats, plasma zinc remains the best available routine marker. There is no widely available, better-validated alternative for clinical use; red cell zinc and hair zinc have both been proposed and neither performs well enough to displace it.

Why zinc is measured

Zinc is tested to confirm suspected deficiency in people whose intake, absorption or losses put them at risk, and to monitor supplementation where a doctor has started it.

Zinc testing is most useful when there is a specific reason to suspect deficiency. In a well-nourished adult with no risk factors and no symptoms, the pre-test probability is low and a borderline result is more likely to generate confusion than insight.

  • Poor dietary intake — strict vegan and vegetarian diets, where phytates in grains and legumes also reduce absorption.
  • Malabsorption from coeliac disease, Crohn's disease, short bowel or bariatric surgery.
  • Chronic diarrhoea or high-output stoma, which cause substantial zinc loss.
  • Chronic liver disease and chronic kidney disease.
  • Alcohol use disorder, which reduces absorption and increases urinary loss.
  • Persistent skin lesions, poor wound healing, recurrent infections, unexplained hair loss, or altered taste or smell.
  • Pregnancy and lactation, where requirements rise.
  • Monitoring during high-dose supplementation, because prolonged high-dose zinc induces copper deficiency.

What a high zinc result can mean

Elevated plasma zinc is almost always the result of supplementation, and sustained excess is clinically relevant mainly because it interferes with copper absorption.

Dietary zinc excess essentially does not occur. A high result in practice means supplements, lozenges taken in quantity, or occasionally occupational exposure to zinc fumes. Haemolysis of the sample can also falsely raise the number, because red cells are zinc-rich.

The reason this matters is copper. Zinc induces metallothionein in the intestinal lining, which binds copper and prevents its absorption. Sustained intakes above the Australian upper level — 40 mg per day for adults, as set in the NHMRC Nutrient Reference Values — can produce copper deficiency over months, presenting as anaemia unresponsive to iron, neutropenia, and in prolonged cases a myelopathy that may not fully reverse.

Acute high-dose zinc ingestion causes nausea, vomiting, abdominal cramps and a metallic taste. If your result is high and you are taking a supplement, that is a conversation to have with your GP or pharmacist about dose and duration rather than something to adjust indefinitely on your own.

What a low zinc result can mean

A low plasma zinc suggests deficiency, but must be interpreted alongside albumin and inflammatory markers before it is accepted at face value.

The first question with any low zinc is whether it is real. Check the albumin: if albumin is low, part of the zinc fall is a carrier-protein effect. Check CRP: if inflammation is present, plasma zinc will be depressed regardless of stores, and repeating the test after recovery is more informative than acting on the acute number.

Where deficiency is genuine, the clinical picture usually gives it away. Zinc-responsive features include a characteristic dermatitis around the mouth, nose and anogenital area, poor wound healing, hair thinning, blunted taste and smell, recurrent infections, and in children slowed growth. Severe inherited deficiency (acrodermatitis enteropathica) presents in infancy and is managed by specialists.

The right response to a genuinely low result is to identify the cause — intake, absorption or loss — and correct that, with supplementation as directed by a doctor and for a defined period rather than indefinitely. Because zinc and copper compete, prolonged supplementation without medical oversight trades one deficiency for another.

Who should consider testing

Zinc testing is most informative for people with a risk factor for deficiency or with symptoms that zinc deficiency could plausibly explain.

If you are testing because of symptoms, add albumin and CRP to the request so the result can actually be interpreted. A zinc number without them is much harder for your GP to act on.

  • Vegans, vegetarians and people with restricted or low-protein diets.
  • People with coeliac disease, inflammatory bowel disease, or after bariatric or bowel surgery.
  • People with chronic diarrhoea, a high-output stoma, chronic liver disease or chronic kidney disease.
  • People with alcohol use disorder.
  • People with unexplained poor wound healing, recurrent infection, hair loss or altered taste.
  • People taking high-dose zinc long term who want to check for excess and consider copper status.

Frequently asked questions

Can I get a zinc blood test without a referral in Australia?
Yes. Privately requested pathology does not require a referral from your own GP. It is not Medicare-rebatable, so the full private fee applies.
Do I need to fast for a zinc test?
A fasting morning sample is preferred. Zinc falls after meals and through the day, so timing affects which reference interval applies.
Should I stop zinc supplements before testing?
If you are testing to find your baseline, withholding supplements for at least 24 hours is usual — but do not stop anything a doctor prescribed without asking them first.
Why does my report include albumin and CRP?
Because both change plasma zinc independently of your intake. Low albumin lowers measured zinc, and inflammation redistributes zinc away from plasma. Without them, a low result cannot be interpreted confidently.
Is hair zinc testing useful?
No. Hair mineral analysis is not a validated measure of zinc status and is not used in Australian clinical practice.
Can too much zinc be harmful?
Yes. Sustained intake above the NHMRC upper level of 40 mg per day for adults can cause copper deficiency, which may present as anaemia, low white cell counts or neurological problems. Discuss long-term supplementation with your GP.
