1. About these terms
These terms apply when you use the FORM Australia website or buy an Australian blood-testing service from HIS FORM PTY LTD (we, us or FORM). By ordering, you agree to these terms. The service is available only where we can arrange collection in New South Wales or the Australian Capital Territory.
2. The service
Your order covers the tests listed for your selected panel, a pathology request made by an Australian-registered doctor, collection through the stated pathology provider, and the report or review inclusions shown at purchase.
FORM provides testing and general information. Website content and reports do not replace emergency care, a diagnosis, or medical advice from your treating practitioner. Seek urgent medical care for urgent symptoms.
3. Eligibility and accurate information
You must be at least 16, provide accurate identity and health details, and use your own order unless we agree otherwise. A doctor may decide that a requested test is inappropriate or requires further information. We may cancel and refund an affected test when it cannot appropriately be requested.
4. Prices and payment
Australian prices are shown and charged in AUD. The total shown at checkout is the amount payable, subject to any clearly disclosed delivery or third-party charge. We do not charge the Bali service fee on Australian orders.
5. Collection and results
Collection is available only at participating Laverty Pathology centres in NSW and the ACT. Centre hours and appointment requirements can change; confirm with the centre before attending. Processing times are estimates, not statutory rights. A repeat collection may occasionally be required for clinical or technical reasons.
6. Cancellations, refunds and statutory consumer rights
Our Australian Refund Policy forms part of these terms. Nothing in these terms excludes, restricts or modifies rights or remedies that cannot lawfully be excluded under the Australian Consumer Law.
7. Privacy
We handle personal and health information as described in our Australian Privacy Policy. You must provide the information needed for a doctor and pathology provider to request and perform the tests.
8. Acceptable use
You must not misuse the site, interfere with its security, submit another person's information without authority, copy protected content at scale, or use the service unlawfully.
9. Liability
Where the law permits, we are not responsible for indirect or consequential loss caused by circumstances outside our reasonable control. Any limitation applies only to the extent permitted by law and does not affect non-excludable statutory consumer rights.
10. Changes and governing law
We may update these terms prospectively. The version accepted when you order applies to that order. These terms are governed by the laws of Australia and the applicable laws of the state or territory connected with the service.
