Can I order now?

Where can I have my sample collected?
Collection is available at participating Laverty Pathology centres in NSW and the ACT. Check Laverty's centre locator for current hours and appointment requirements before attending.
What do I bring?
Bring the pathology request supplied after your order and follow any preparation instructions included with it.

Can I build my own panel?
Yes. The custom builder lets you select from the biomarkers currently available in the Australian catalogue, with AUD pricing updated as you build.
Will I be charged in AUD?
Yes. The Australian storefront displays and charges its listed prices in AUD through Shopify checkout.
Are the listed prices final?
Yes. The prices displayed for the Australian panels are the current prices charged at checkout.
Do private health funds or Medicare provide a rebate?
FORM Australia tests are privately paid and are not Medicare-rebatable. An Australian-registered doctor requests the selected tests and provides oversight of the results. Private health cover varies; check directly with your fund.

When will I receive results?
Timing varies by test and collection date. We will notify you when your results and FORM report are ready.
What does the report include?
The report shows laboratory reference ranges and plain-English marker information. It does not replace individual medical advice from your treating doctor.
