When does FORM Australia launch?
We're finalising our Laverty (Healius) partnership. There's no fixed launch date — join the waitlist and we'll email you as soon as orders open.
Why publish prices before launch?
So you can compare. The AUD prices on the site are indicative — they're what we're targeting at launch. Final pricing will be confirmed once our Laverty terms are signed.
Will I get launch pricing if I'm on the waitlist?
The AUD prices shown today are indicative. We'll email waitlist signups the confirmed price before opening orders so you can decide whether to proceed — no launch pricing or booking slot is promised in advance.
Collection & logistics
How does the blood draw work in Australia?
After you order, we email a pathology collection request PDF. You walk it into any Laverty (Healius) pathology centre — there are hundreds across every state and territory. Most centres take walk-ins.
Do I need an appointment?
Most Laverty centres accept walk-ins. Some metro centres also offer online appointment booking. Either way, you bring the FORM collection request PDF.
What do I need to bring to the collection centre?
The FORM collection PDF (on your phone is fine), and your Medicare card if you have one. Some markers require fasting — your PDF will say which.
How long does collection take?
Around 10 minutes from sign-in to leaving. Walk-in wait times vary by centre and time of day.
Can I collect at a centre other than Laverty?
At launch we route exclusively through Laverty. This keeps quality and turnaround consistent. We'll review additional pathology partners post-launch.
Panels & biomarkers
Why are the Australian panels bigger than the Bali ones?
Australian pathology per biomarker is cheaper than our Bali partner, so the same dollar buys you a deeper panel. Each Australian tier carries more markers than the equivalent Bali tier at a comparable AUD cost.
Can I build my own panel?
Yes. Our custom builder lets you pick from 50+ biomarkers individually. Indicative AUD pricing updates live as you build.
Do you test hormones beyond testosterone?
Yes. From the Pro panel onward we cover the full male hormone axis: total + free testosterone, SHBG, DHEA-S, cortisol, LH, FSH, estradiol, prolactin. Ultra adds DHT.
Pricing & rebates
Do private health funds rebate any of this?
Medicare: pathology you request yourself is not Medicare-rebatable. A Medicare benefit is only payable when a treating practitioner requests the test and the request meets the relevant MBS item criteria. FORM panels are privately paid in full, and private health funds generally do not cover self-requested pathology either.
Will I be charged in AUD?
Yes. The Australian storefront prices and charges in AUD via Shopify when orders open.
Are the prices on the site final?
No — they're indicative. The page clearly marks them as such. Final pricing is locked in when our Laverty terms are signed; waitlist signups get the indicative price as a ceiling.
Results & follow-up
How fast are results?
Most panels are processed within 7–14 days. You'll get an email when your expert-reviewed PDF is ready in your FORM dashboard.
Do you prescribe treatment?
No. FORM is a pathology testing service. Every package includes an expert-reviewed PDF report with personalised insights and clear next steps on which results are worth discussing further — we don't prescribe medication or design a treatment protocol. If your results warrant follow-up, you take the report to your own GP or specialist.
Is the report reviewed by a doctor?
Yes. Our Medical Director, Dr. Nikola Topalovic (MD, PhD), reviews the interpretation on every report.
Indonesia & other markets
Will Indonesia still be available?
Yes. FORM Indonesia operates independently with a certified nurse home-draw model in Bali. Visit gethisform.com for the Indonesian storefront.
Are you launching in other countries?
